Other ways to qualify

  • Comparable conditions. Conditions of the same kind or class as, or comparable to, those listed above.
  • Terminal conditions. A terminal diagnosis made by a physician other than the one certifying you.
  • Chronic nonmalignant pain. Pain that meets the state's criteria, as determined by your physician.
